PARENTAL PERMISSION TO PLAY WAIVER
I, THE PARENT OR GUARDIAN OF THE ABOVE NAMED APPLICANT, GIVE APPROVAL TO MY CHILD’S PARTICIPATION IN ALL ACTIVITIES OF THE KINGSTON THUNDER BASEBALL CLUB PROGRAMS. I ASSUME ALL RISKS AND HAZARDS INCIDENTAL TO SUCH PARTICIPATION INCLUDING TRANSPORTATION TO AND FROM ALL ACTIVITIES.
I DO HEREBY WAIVE, RELEASE, ABSOLVE, INDEMNIFY, AND AGREE TO WITHHOLD THE KINGSTON THUNDER BASEBALL CLUB, THE ORGANIZERS, COACHES, SPONSORS, OFFICIALS, SUPERVISORS, OTHER PARTICIPANTS, AND APPOINTED PERSONS TRANSPORTING MY CHILD TO OR FROM PROGRAM ACTIVITIES FROM ANY CLAIMS ARISING OUT OF INJURY TO MY CHILD INCIDENTAL TO SUCH PARTICIPATION, EXCEPT TO THE EXTENT AND AMOUNT COVERED BY ACCIDENT AND /OR LIABILITY INSURANCE HELD BY THE KINGSTON THUNDER BASEBALL CLUB.
I FURTHER AGREE THAT IN MY ABSENCE THE DESIGNATED LEAGUE OFFICERS, AND/OR TEAM COACHES SHALL HAVE AUTHORITY TO TAKE ACTION, AS DEEMED NECESSARY, TO PROVIDE OR RENDER IMMEDIATE MEDICAL ATTENTION TO THE ABOVE NAMED PARTICIPANT DUE TO SUDDEN ILLNESS OR INJURY INCIDENTAL TO, OR OCCURING DURING HIS /HER PARTICIPATION.
I AGREE TO PAY THE REGISTRATION FEE, TO ADHERE TO LEAGUE RULES AND THE KINGSTON THUNDER BASEBALL CLUB CODE OF CONDUCT AND TO RETURN IN GOOD CONDITION ANY EQUIPMENT OR UNIFORM ISSUED TO MY CHILD, AND TO FURNISH, IF REQUESTED, A CERTIFICATE OF BIRTH FOR MY CHILD.

This Code of Conduct identifies the standards of behaviour which is expected of all members of the KTBA. This Policy is applicable to all players, parents, guardians, managers and coaches, officials, volunteers, directors, officers, committee members, convenors, team managers, administrators and employees involved in KTBA activities and events.
The KTBA is committed to providing an environment in which all individuals are treated with respect. Members and participants of the KTBA shall conduct themselves at all times in a manner consistent with the values of the KTBA, which include fairness, integrity and mutual respect, and the promotion of fair play, sportsmanship, and player development.
Members and participants of the KTBA shall not engage in any activity or behaviour which interferes with a competition, or with any player or team’s preparation for a competition, or which endangers the safety and well being of others. At all times, KTBA members and participants shall refrain from comments, or behaviours, which are disrespectful, offensive, abusive, racist or sexist.
Specifically, during the course of all KTBA activities and events, members shall avoid behaviour which brings KTBA into disrepute, including but not limited to, the following:
- - Verbal abuse to Players, Managers & Coaches, and Game Officials.
- - Threatening, or pushing, Players, Managers & Coaches, and Game Officials.
- - Consumption of alcoholic beverages, and the use of tobacco or illegal drugs in the ballpark, before, during, or after a game or practice.
- - Disruption of a game by causing arguments or altercations with other Spectators, Players, Managers & Coaches, and Game Officials.
Failure to comply with this Code of Conduct may result in disciplinary action in accordance with the Discipline Policy of the KTBA (located on the KTBA website). Such action may result in the member losing the privileges which come with membership in the KTBA, including the opportunity to participate in KTBA activities and events, both present and future.
